Diabetes is a chronic-progressive disease that requires continuing medical care and patient self-management education to prevent acute complications and to reduce the risk of long-term complications.

Your healthcare team and support network are there to help you, but managing your diabetes is up to you. The goal of Diabetes Self-Management Education (DSME) is to provide educational information for you, and your family, to learn how to manage diabetes.

The benefits of education include:

  • Improved glucose control
  • Feeling better and having more energy
  • Decreasing your risks of diabetes complications

To develop an individualized plan for the diabetic patient, the DSME team evaluates several factors regarding each participant’s ability to engage in diabetes self-care. They are:

  • Nutritional planning of individual and lifestyle needs
  • Health status
  • Attitudes, beliefs, experience and desire to participate in diabetes education
  • Psychosocial status
  • Literacy and learning style
  • Cultural and lifespan issues
  • Personal goals and small steps
  • Self-care skills and access to resources

Diabetes education is reimbursed as part of the Medicare program as overseen by the Center for Medicare and Medicaid Services. Many private insurances cover the service as well.
