Firelands Regional Medical Center’s Center for Coordinated Care is proud to offer a Diabetes Care Center. Our comprehensive program connects patients with a multidisciplinary team of diabetes experts who are committed to helping patients feel confident managing their diabetes using the latest treatments, technology, and information available. The diabetes care center offers diabetes specialists to help manage your condition, as well as a Diabetes Self-Management Education program to help you live and thrive with diabetes.

Our talented team of compassionate specialists includes board-certified providers, diabetes care and education specialists, weight loss specialists, nurse practitioners, and specialty dietitians and pharmacists. Our clinicians work alongside you and your primary physician to add a layer of specialized support to improve the quality of treatment and your quality of life with diabetes so that you can focus on what’s most important to you.
Living with diabetes can be very challenging, whether you have prediabetes, Type 1 or Type 2 diabetes. No matter where you are in your journey, we can help you have the resources and tools to live your best life with diabetes. Our multidisciplinary approach can help you get access to the latest and most advanced medical care, technology, and education to prevent and manage long-term complications associated with diabetes.

Diabetes self-management education program
Our clinicians believe education empowers you and your family to manage your disease, live an active lifestyle and prioritize your health. All persons with diabetes will benefit from education. Whether you are newly diagnosed with diabetes or have years of experience, our goal is to empower you with the most current information to help you have confidence navigating this lifelong journey.
Certified diabetes care and education specialists (CDCESs) are available for one-on-one and/or group support for many topics including:
- Diabetes medication-related education
- Blood glucose monitoring and continuous glucose monitoring
- Insulin injection education
- Insulin pump training and troubleshooting
- Counting carbs and nutritional guidance
A referral from your primary diabetes provider is required to join the DSME program.
Who should consider diabetes self-management education?
This program is for individuals diagnosed with diabetes and their families. It’s designed to provide educational information and self-management skills while encouraging patients to live an active lifestyle and prioritize their health.
The diabetes self-management education program is beneficial to those with diabetes (Type 1 or Type 2) who are newly diagnosed OR longstanding.
What are the benefits of the diabetes self-management education program?
Over time, diabetes patients who participate in this program may experience:
- Improved glucose (blood sugar) control
- More energy and overall improved health
- A decrease in the risk of diabetes complications
Do I need a doctor's referral?
Yes. A diabetes patient must have a referral from a provider. If you call and would like to set up an appointment, a referral request is sent to your PCP. If you do not have a PCP, the Firelands Center for Coordinated Care can assist you with establishing with a primary care physician.
The Diabetes Care Center is committed to providing the highest quality of care for patients with diabetes, diabetic devices, and metabolic disorders, including:
- Prediabetes
- Diabetes: Type 1 diabetes, Type 2 diabetes, Mellitus, and those using insulin pumps or continuous glucose monitoring systems.
