discharge Discharge process

After your stay at Firelands Regional Medical Center is complete, a discharge specialist will contact you. The discharge specialist will review your discharge instructions, answer any questions or concerns, and talk with you about your recent stay at Firelands Regional Medical Center.

The discharge specialist will also assist you with:

  • Understanding your discharge instructions, including home care
  • Reviewing new medications, including why the medication was prescribed, potential side effects and when you should take your medication
  • Obtaining medications if you are unable to afford the medication or if you are waiting on prior authorizations
  • Reviewing the importance of your follow-up appointments and assist with arranging transportation if needed
  • Assisting with scheduling appointments at Firelands Regional Medical Center - Center for Coordinated Care, if needed
  • Arranging home health care, if needed

Coping with the unexpected illness of a loved one can be overwhelming. If that illness occurs in an unfamiliar place, many people find themselves without their usual resources to handle such an emergency situation.

At Firelands Regional Medical Center, we realize that many of our patients and their loved ones may be visiting the area while on vacation or during a holiday when the hospitalization occurs. In such circumstances, our help desk is dedicated to addressing the needs of our out-of-town guests.

Visit our Information Desk at our Main Campus or call us to learn about hospital information such as dining, visiting hours, and pharmacy information. We can also suggest local resources such as groceries, laundry, lodging, notary, and transportation. If you have any question, we will help you find an answer. We want your stay in our area to be as easy and comfortable as possible.

We are committed to providing access to healthcare for all persons, regardless of their ability to pay. Family Health Services

Family Health Services is a non-profit, Federally Qualified Health Center in Sandusky that provides primary care for adults, children and infants at their Hayes Avenue location, and primary care for adults at their E. Water Street location. Family Health Services accepts Medicare, Medicaid, and insurance from the majority of providers. For patients who have no insurance, services are offered on a sliding fee scale to assist people in affording quality health care.
