Patient Forms
You can download, print and fill out some of your patient registration forms prior to your appointment to reduce wait times. Downloadable patient forms include patient registration for adults, patient registration for pediatrics, privacy notice acknowledgement form, authorization for release of medical information, and the Firelands Physician Group financial policy.

Medical Records
A patient, or his/her legal representative, may inspect and/or obtain a copy of their medical records, or have copies of medical records sent to another facility.

Financial Assistance
Firelands Physician Group realizes not all patients have the means to pay for services; programs such as Financial Hardship are offered with the information and application being given to the patient/guarantor when financial hardship is indicated. Financial hardship is considered charity care. The program will be offered to those requesting assistance and who qualify.

Advance Directives
Advance directives are legal written documents that indicate what an individual wants regarding care and treatment if he or she is not able to speak for himself or herself. There are three documents: a healthcare power of attorney, a living will and a do-not-resuscitate order. All must be signed by the competent patient and witnessed or notarized.
